When it is needed
When participants explain the event mechanism differently, materials contain contradictions or the technical part of the commission's findings needs checking.
What we examine
- Workplace, equipment and items involved in the event.
- Photographs, video recordings, diagrams and inspection records.
- Documents describing the brief, working procedure and intended protective measures.
Materials to prepare
- Investigation materials, including appendices to the report and records.
- Original photographs and video with their source and time of acquisition.
- Equipment technical passport, manual and maintenance documents.
- Instructions, work permits, process sheets and information about scene changes.
Sample questions
- Which sequence of technical events can the materials provided establish?
- Which hazardous factors were present in the work area considered?
- Which protective devices and organisational measures were intended for this operation?
- Is the incident mechanism described in the documents consistent with the recorded scene?
- Which missing information limits checking of the explanation proposed?
How the work is carried out
- We will define the operation, period and explanations to be checked.
- We will compare primary recordings, technical documentation and results of the inspection available.
- We will substantiate established circumstances and separate them from assumptions.
Result
A specialist's report describing source data, explanations checked, technical factors and limits of findings.
What affects the conclusions
The site's current condition may differ from its condition on the event date. The examination does not replace a commission's investigation or establish participants' legal culpability.
Frequently asked questions
Can we start without a site visit?
Yes. Document analysis can establish which questions can be resolved from the available materials and whether inspection is needed. The final scope is agreed after familiarisation.
What if the equipment has already been repaired?
Preserve repair information, removed parts, photographs and records. We will identify the object's alteration in the examination and take it into account when assessing possible findings.
