Inspection materials and a yellow hard hat in industrial premises

Break the causal finding into individual claims

A report may link an occupational accident to a faulty interlock, unexpected mechanism movement or a part failure. The reviewer checks not only the final statement but also the steps leading to it: which movement was established, where the energy came from, how contact became possible and what substantiates the event sequence. Equipment damage and an explanation of its origin require different grounds.

It helps to set out several technical claims in sequence and identify a source for each. For example, a recording may substantiate the protective door's position, a test record the sensor's condition and a control log the machine's mode. If two neighbouring links are connected only by an assumption, establish whether the author identified it and how much the final answer depends on it.

Choose explanations that genuinely differ in observable indicators

An alternative explanation is useful when it accounts for observations and can be checked. For equipment movement, possible explanations include a normal operating cycle, residual movement after a stop command, a technical fault or action by another line component. This is not a list of ready-made causes for every case. Selection depends on the design, event materials and circumstances still disputed.

For every substantive explanation, expected indicators are identified: signal sequence, trace characteristics, part positions or agreement with timestamps. It is then checked whether the author looked for them and how the result was assessed. The phrase “no other faults were found” is insufficient without identifying the components inspected and methods used. However, absence of information about an explanation does not itself prove it.

Check whether the defect existed before the event

A broken fastening or deformed casing may have contributed to the incident, arisen during the impact itself or changed in subsequent actions. The review establishes how the author determined the sequence. Initial images, repair documents, the condition of mating parts and damage examination results are compared. A general photograph does not always establish when failure occurred or its original mechanism.

Testing an already repaired object and comparison with another machine need particular attention. An analogue may demonstrate the operating principle, but differences in variant, settings, load and wear limit transfer of the results. The reviewer checks whether these differences are listed and the comparison justified. Similar appearance does not substantiate identical technical condition of two objects.

Check the chronology against sources with known limitations

Camera, controller and registration log clocks may run independently. Command and movement sequences are therefore checked with regard to synchronisation, timestamp accuracy and the intervals actually observable. One frame does not show what happened between frames. A recorded stop message does not necessarily mark the moment the mechanical part actually stopped.

Participant accounts are compared with technical data while preserving the distinction between an observed action and its explanation. The phrase “the machine started by itself” may convey an observer's impression but requires clarification of mode and control signals. If sources contradict one another, the report must explain its choice of basis. Simply preferring a convenient account does not resolve the contradiction or restore a missing section of chronology.

An observation must show its effect on a specific answer

A substantive review identifies the page, claim being checked, data used and unresolved technical alternative. It then explains the consequence: additional component examination, clarification of the initial mode or a less categorical answer is required. Not every omission changes the result. Sometimes other independent indicators are sufficient, and the reviewer must show this as clearly as any error found.

A review may substantiate a causal explanation, recognise it as conditional or establish that the materials are insufficient to choose between explanations. It need not propose a single new cause when source data have been lost. Checking technical reasoning does not replace an investigation commission's work, medical examination of injuries or legal assessment of liability; a special assessment of working conditions (SOUT) and industrial safety expert examination also have distinct purposes.
